How to use this template

  1. Ask for facts with dates and a location rather than opinions.
  2. Require a signature so the board is not acting on anonymous claims, while offering confidentiality where possible.
  3. Include a requested-outcome field so expectations are clear.
  4. Log each form with a reference number and a closing date.
  5. Reply to the owner in writing with what was done, even if the answer is nothing.

You can accept them, but acting on an anonymous complaint alone is risky. Verify independently before any notice.

Is the complainant's identity disclosed?

Not by default. In a hearing the accused owner may be entitled to know the basis of the complaint; state law varies.

How quickly should the board respond?

Acknowledge within a few business days and give a realistic timeline for a decision.

Can an owner complain about the board?

Yes. Route those to the manager or a designated director who is not the subject of the complaint.
